Preoperative Evaluation (Pre-Op-Eval)

Are you planning on surgery and your surgeon wants you to get a clearance from your physician?

Eliminate the hassle of having to find a PCP in the area and pay hundreds of dollars in lab costs. Evolve Wellness and Aesthetics offers you speedy preoperative clearance evaluations that will get you approved for your procedure. We don’t require you to make me your PCP if you don't have one, we use LabCorp, CPL, and Quest laboratories!

Schedule preoperative evaluation appointment at our clinic. This is a longer appointment than your usual follow-up visit and gives your physician enough time to do a complete physical exam prior to any type of surgery involving general anesthesia.

When to Schedule your Appointment?

Schedule your preoperative visit at least one to two weeks prior to your surgery to give us enough time to address any abnormal findings on a physical exam or tests. This is to ensure your safety and to reduce complications during and after your surgery.

All appointments will require a deposit based on the appointment booked, payment will be applied to your services. 
  • ALL DEPOSITS ARE NON-REFUNDABLE
  • DEPOSIT MUST BE PAID BEFORE APPOINTMENT CAN BE CONFIRMED
  • MORE THAN 15 MINUTES LATE APPOINTMENT WILL BE CANCELED
  • NO CALL NO SHOW YOU WILL NOT BE ALLOWED TO BOOK ANY FUTURE APPOINTMENTS.

How it Works?

We provide the surgical clearance after a thorough examination, blood work, EKG, CXR and other tests deemed appropriate by your doctor.Mammograms and CXRs can be referred to a local imaging center. 

If you develop a medical problem such as an infection after we have cleared you for surgery, please let both your surgeon and physician know immediately, so that they can treat your problem before surgery and make adjustments to your surgery schedule if needed.
